Benefits

This Plan provides short-term disability benefits, accidental death and dismemberment benefits and life insurance benefits for Employees only, and medical, prescription, dental, and vision benefits for Employees and Dependents.

Claims arising from the Plan for medical and dental benefits for Employees and Dependents and the Short-Term Disability benefits for Employees are paid directly from Trust assets. The Trust has entered into a contract with Anthem Blue Cross Preferred Provider Organization that can be used by Employees and Dependents enrolled in the Self-Funded Medical Indemnity PPO Plan for Medical Benefits.
